Patient record

Review a patient's details and coverage, and deactivate, merge or ask to delete a record.

The Patient record holds one patient's demographics and insurance. It opens from Schedule Ride or from the Patients search, with a link back to where you came from. Its address and links use a short reference, never the patient's name.

Everyone who can see patients can read a record. Editing, and the record actions below, belong to Facility Admins.

Demographics

  1. Read the card: Full name, Date of birth, Sex, Height, Weight, MRN, Mobile phone, Preferred language and SMS consent.
  2. A Facility Admin selects Edit, changes what is wrong and selects Save changes. First and last name are required.
  3. Select Cancel to leave without saving.

The audit log records which fields you changed, never their values.

Coverage

The Coverage card shows the patient's primary and secondary insurance: Class, Plan and Member ID.

  1. A Facility Admin selects Edit on a coverage.
  2. Choose the Class (Medicare, Medicaid or Private), enter the Plan and Member ID, and select Save insurance.
  3. To take one off, select Remove and confirm. It leaves the patient's record, not only one ride.

Coverage fills in step 4 of Schedule Ride the next time you book this patient.

Record actions

The Record actions card is for a duplicate, a patient you no longer book for, or a patient who asked to be removed. Each action is recorded in the audit log by field name, never by value.

Deactivate or reactivate

  1. Select Deactivate, then confirm with Deactivate patient.
  2. Schedule Ride stops offering the patient. Rides already booked keep them, and nothing is erased.
  3. Find them again on Patients with Include deactivated. Select Reactivate to offer them again.

Request deletion

  1. Select Request deletion.
  2. Pick a reason: The patient asked to be removed, Created in error or Other, discussed with HealthyRides.
  3. Select Send deletion request. A HealthyRides administrator reviews it, and the record stays as it is until then.
  4. The card shows the request's reference and status. If it is approved, the patient's name, date of birth, MRN, phone, addresses, notes, diagnoses and member IDs are erased from the record and its rides. Ride times, statuses, provider, payer class and grades are kept, with no patient detail.
  5. If it is rejected, the card shows Reason from HealthyRides. Nothing is erased, and you can ask again.

Merge a duplicate

  1. Select Merge a duplicate.
  2. Search for the duplicate by name, date of birth or MRN and select Pick as duplicate. The record you are on is the one that stays.
  3. Read the comparison. If the two differ, the screen says in which fields. Merge only if they are the same person. This record's details are kept.
  4. Select Merge into this record, or Pick another or Cancel.

Common questions

Why can't I see Edit or Record actions?

Only Facility Admins can change a patient. Ask one to make the change.

Does deactivating erase anything?

No. It only hides the patient from booking. Use a deletion request to erase.

Names stay out of links so they are not read aloud by a screen reader.
